I don't really know why he is referred to as Morpheus by Smith, it is possible that because of his "popularity" to the bluepills as Morpheus led to the Machines just refering to him as thus. Or maybe, due to his talents, he erased or altered his name in the Matrix.

Yeah, they called him the anomaly because they were talking to each other about him. Smith refused to call him Neo, even at the end, the Oracle called him Neo through the Smith clone.
Interestingly, why would he call Morpheus by his handle while addressing him? "The great Morpheus, we meet at last..." Did he respect Morpheus? Not likely. He tortured him but still called him Morpheus while addressing him and not his bluepill name.
